Support & Lead Patients

Support & Lead Patients

Support & Lead Patients

 As a Direct Support Supervisor, you will lead a team of Direct Support Professionals (DSPs) to ensure high-quality, person-centered care for individuals with intellectual and  developmental disabilities. You’ll oversee daily operations of assigned programs or residences, support staff development, and uphold compliance with DDS regulations—all while promoting dignity, independence, and inclusion for the people we serve.

 

Key Responsibilities

  • Supervise, train, and support a team of DSPs 
  • Monitor daily operations and ensure a safe, supportive environment 
  • Coordinate services, appointments, and individualized goals 
  • Maintain accurate documentation and meet DDS compliance standards 
  • Serve as a liaison with families, guardians, and external providers 
  • Respond to emergencies and provide on-call support as needed

 

Qualifications

  • High school diploma or GED required; Associate’s or Bachelor’s preferred 
  • 2+ years experience in human services or disability support 
  • 1+ year of supervisory experience preferred 
  • Valid driver’s license & reliable transportation 
  • Strong communication, leadership, and organizational skills
     

Coordinate with Care

Support & Lead Patients

 The Direct Support Coordinator – Services plays a vital role in ensuring the delivery of high-quality, individualized services for people with intellectual and developmental disabilities. This position coordinates daily support plans, tracks progress toward goals, and ensures services are aligned with DDS requirements and person-centered principles. 

 

Key Responsibilities

  • Develop and coordinate individualized service plans (ISPs) 
  • Work closely with individuals, families, and support teams to monitor progress 
  • Ensure all services meet DDS standards and documentation requirements 
  • Collaborate with residential, vocational, and clinical providers 
  • Maintain accurate records, reports, and billing information 
  • Participate in ISP meetings and support plan reviews
     

Qualifications

  • Associate’s or Bachelor’s degree in Human Services, Social Work, or related field preferred 
  • 2+ years of experience supporting individuals with developmental disabilities 
  • Strong understanding of DDS systems and person-centered planning 
  • Excellent communication and case management skills 
  • Valid driver’s license and reliable transportation
     

Care & Support

Care & Support

 As a Direct Support Caregiver, you will provide compassionate, hands-on support to individuals with intellectual and developmental disabilities. Your role is to assist with daily living tasks, promote 

independence, and help individuals achieve their personal goals in a safe and respectful environment. 

 

Key Responsibilities

  • Support individuals with daily living skills (hygiene, meals, routines) 
  • Assist with community outings, appointments, and recreational activities 
  • Foster independence, choice, and dignity in every interaction 
  • Document daily activities and progress toward personal goals 
  • Ensure a safe, clean, and supportive home environment 
  • Work collaboratively with the care team, families, and supervisors
     

Qualifications

  • High school diploma or GED 
  • Experience in caregiving, human services, or healthcare preferred 
  • Compassionate, reliable, and patient demeanor 
  • Ability to lift, bend, and assist with mobility as needed 
  • Valid driver’s license and reliable transportation preferred
